Tiefenfluorid deep fluoridation: how it works and when to use it

Tiefenfluorid deep fluoridation is a two-solution system: the first solution, containing fluorosilicate, followed by a calcium hydroxide solution, reacts directly within the enamel and the dentinal tubules, where calcium and magnesium fluoride crystals form, protected from abrasion. This creates a long-lasting fluoride and calcium reservoir. The manufacturer, Humanchemie (Germany), indicates the product for desensitization of tooth necks, mineral fissure sealing and caries prevention, including in patients with brackets.

How deep fluoridation differs from conventional fluoridation

Conventional topical fluoridation (varnishes, gels, solutions) leaves fluoride compounds mainly on the tooth surface. Tiefenfluorid works on a different principle: the reaction takes place not on the surface but deeper, within the enamel structure and the dentinal tubules. The crystals deposited there are protected from abrasion, which is why the manufacturer emphasizes its long-lasting reservoir effect. The deposits left after treatment are invisible to the naked eye, so esthetics are not compromised.

Composition and mode of action

  • First solution (Touchierlösung): magnesium hexafluorosilicate, sodium fluoride (stabilizer), distilled water.
  • Second solution (Nachtouchierlösung): highly dispersed calcium hydroxide, methylcellulose, distilled water.

The first solution penetrates the enamel and dentinal tubules, and the second reacts with it to form fine calcium and magnesium fluoride crystals. Thanks to their low viscosity, the solutions easily reach interproximal areas, the areas around brackets and other hard-to-reach sites.

Indications and repeat intervals

Humanchemie specifies the following indications and repeat schedules:

Indication Repeat schedule per manufacturer
Sensitive tooth necks (desensitization) Repeat 1–2 times every 8–14 days, then 1–2 times a year
Mineral fissure sealing Repeat after 8–14 days, then twice a year
Caries prevention, including hard-to-reach areas At least once a year
Patients with brackets At least twice a year

The manufacturer also states that the product can help reduce white spot lesions.

How the procedure is performed

  1. Clean the tooth surface and dry it relatively.
  2. Apply the first solution thoroughly with a cotton pellet or brush.
  3. Do not rinse: immediately apply the second solution with a separate cotton pellet or brush.
  4. Only then rinse with water.
  5. The patient can eat and drink straight away; no waiting or curing is required.

Tiefenfluorid, balance or junior?

  • Tiefenfluorid: the classic, fragrance-free version; the manufacturer describes it as the better option for allergic patients.
  • Tiefenfluorid balance: the second solution additionally contains copper compounds. According to the manufacturer, they neutralize the mildly acidic inflammatory environment and have an antibacterial effect, so this version is intended for patients at increased risk of periodontitis or with signs of gingival inflammation.
  • Tiefenfluorid junior: with xylitol and natural strawberry flavor, sugar-free; the fluoride concentration is the same as in classic Tiefenfluorid. Intended for children and for adults who dislike the taste of the classic solution.

All three versions are supplied as 5 ml + 5 ml (trial pack) or 20 ml + 20 ml kits. Prices incl. VAT are shown on the product page.

Which pack to choose

The 5 ml + 5 ml kit is convenient if you are just starting to offer deep fluoridation in your clinic or want to try several versions, for example classic and junior. The 20 ml + 20 ml kit suits clinics where the procedure is performed regularly: orthodontic, pediatric dentistry or prevention practices. Humanchemie also supplies the second (Nachtouchier) solution separately for topping up the Tiefenfluorid system. When choosing a version, consider which patients you see most: if you treat many children, junior is worth having; if many of your patients have periodontal problems, choose balance.

Deep fluoridation or fluoride varnish?

A fluoride varnish, such as Vericom V-varnish Premium, works differently: it leaves a clear film on the tooth that adheres to the surface and releases fluoride. Both products are used to reduce sensitivity and prevent caries, but they differ in form and workflow.

Criterion Tiefenfluorid (Humanchemie) V-varnish Premium (Vericom)
Form Two solutions applied one after the other Varnish in single-use 0.4 ml unit doses with an attached brush
Active ingredients Magnesium hexafluorosilicate and sodium fluoride + calcium hydroxide 5% sodium fluoride, tricalcium phosphate (TCP), xylitol
Site of action Crystals form within the enamel structure and dentinal tubules A clear film on the tooth surface releases fluoride
After treatment Eating and drinking allowed immediately According to the manufacturer's instructions
Repeat schedule Depending on the indication: 1 to 2 times a year after the initial course Every 3–6 months depending on the patient's risk group
Age All age groups; junior version for children From 6 months
Pack 5 ml + 5 ml or 20 ml + 20 ml 5, 50 or 200 pcs

Tiefenfluorid is the right choice when you need a long-lasting reservoir effect deeper in the tissues: for sensitive tooth necks, mineral fissure sealing, patients with brackets and hard-to-reach areas. A fluoride varnish is convenient when you need a quick, hygienic procedure with a single-use dose, especially for young children from 6 months and patients on a regular 3–6-month prevention cycle. The final choice depends on the patient's risk and the clinician's judgment.

Who benefits most from deep fluoridation

  • Orthodontic patients. Plaque is hard to clean around brackets, and the low-viscosity solutions reach these areas. The manufacturer also indicates the product for protecting teeth after bracket bonding (for bonding brackets, IMMT offers the U-Bond Ortho adhesive).
  • Patients with sensitive tooth necks. The crystals are deposited in the dentinal tubules, which is why the product is intended for desensitization.
  • Children and adolescents. Mineral fissure sealing and prevention; the pleasant taste of the junior version makes the procedure easier.
  • Patients at increased risk of periodontitis. For them, the manufacturer recommends the balance version with copper compounds.

The most common mistakes

  • Rinsing between solutions. The second solution must react with the first, so do not rinse in between.
  • One cotton pellet for both solutions. Use a separate pellet or brush for each solution.
  • Skipping the repeat application. For desensitization and fissures, the manufacturer specifies a repeat after 8–14 days; without it, the initial course remains incomplete.

Practical checklist

  • Determine the indication: sensitivity, fissures, prevention or an orthodontic patient; the repeat schedule depends on it.
  • Choose the version: classic (allergic patients), balance (periodontal risk), junior (children, taste).
  • Prepare two separate cotton pellets or brushes, one for each solution.
  • Remember: do not rinse between solutions; rinse only after the second.
  • Note the date of the repeat visit (8–14 days for desensitization and fissures).
  • For patients with brackets, schedule the procedure at least twice a year.

Frequently asked questions

How long does a deep fluoridation procedure take?

The procedure is short: two solutions are applied one after the other without rinsing in between, then the teeth are rinsed with water. No waiting or curing is needed, and the patient can eat and drink immediately.

How often should Tiefenfluorid be repeated?

According to the manufacturer: for desensitization – repeat 1–2 times every 8–14 days, then 1–2 times a year; for fissures – repeat after 8–14 days, then twice a year; for caries prevention – at least once a year, and at least twice a year for patients with braces.

How does Tiefenfluorid balance differ from the classic version?

The second solution of balance additionally contains copper compounds which, according to the manufacturer, neutralize a mildly acidic inflammatory environment and have an antibacterial effect. This version is intended for patients with an increased risk of periodontitis.

Is Tiefenfluorid suitable for children?

For children there is Tiefenfluorid junior with xylitol and strawberry flavor, sugar-free. Its fluoride concentration is the same as classic Tiefenfluorid.

Does a visible layer remain after the procedure?

No. According to the manufacturer, the deposits left are invisible to the human eye, so the product is also suitable for anterior teeth.

When should fluoride varnish be chosen instead of deep fluoridation?

Fluoride varnish such as V-varnish Premium is convenient for quick prevention with single-dose units, is suitable for children from 6 months, and is repeated every 3–6 months depending on the risk group. The final choice depends on the patient's risk and the clinician's decision.
